Failure Root Cause Analysis (RCA)
Root Cause Analysis is a vital process in establishing what caused the failure of the machine or component. Sometimes there are multiple failure modes in a machine or component and the initial cause (root cause) needs to be determined. When the root cause of a failure has been identified, this allows for measures to be introduced to prevent that failure from occurring again.
This is a proactive approach to increasing plant reliability by eliminating the causes of possible failure rather than continuously repairing or replacing machines due to the same faults.
The combination of many years of experience in multiple reliability disciplines along with experience conducting RCA’s, gives Howler technicians great insight when performing a RCA.
